Form Instructions:

1. You will need Microsoft Word in order to fill out the form. If you do not have Microsoft Word, you can use Microsoft Word for free online here.
2. Download the Appropriate Form:
   Training Form for training sessions
   Behavior Assessment Consultation Form for behavior sessions
   It should open as a document in Microsoft Word. If you don’t have Word, you can open it for free online using Microsoft Word here.
3. Enable Editing
   At the top of the form, click “Enable Editing” to allow changes.
4. Enter the Date
   Click on the date field and enter the appropriate date.
5. Complete the Form
   Use the Tab key to move through the rest of the document and fill out all required sections.
6. Save the Completed Form
   Once finished, click Save and rename the file using the following format:
   Your Full Name – Your Dog’s Name – Consultation Date
7. If you are using Microsoft Word online, download the document and save it to your computer.
8. Email the Form
   Send the completed form as an attachment to: info@CarolsCanineTraining.com
